본 연구는 임플란트 상부 구조물 제작을 위한 pick-up 인상 채득시 flag type의 새로운 인상용 코핑을 이용하여 제작된 모형과 기존에 사용되고 있는 인상용 코핑을 사용하여 제작된 모형의 정확성을 3차원적으로 비교하여 새로운 형태의 인상용 코핑을 사용한 인상법의 유용성을 알아보고자 하였다. 좌측은 전부 무치악, 우측은 제 2 소구치가 상실된 상악을 상정하여 아크릴릭 레진 블록으로 기준 모형을 제작하였다. 상악 우측 제 2 소구치, 좌측 중절치, 좌측 견치, 좌측 제 2 소구치, 좌측 제 2 대구치 부위에 외부 육각 연결 방식의 임플란트 고정체(fixture)를 식립하였다. 기준 모형에 사용된인상용 코핑 및 코핑 간의 연결 여부에 따라 세 가지 실험군으로분류하였다. 임플란트 고정체에 새로운 형태인 flag type의 인상용 코핑을 체결한 경우(실험군 Ⅰ), 기존 형태의 원통형 인상용 코핑을 체결한 후 인상용 코핑을 자가 중합 레진으로 연결한 경우(실험군 Ⅱ), 원통형 인상용 코핑을 체결한 후 코핑 사이를 자가 중합 레진으로 연결하지 않은 경우(실험군 Ⅲ). 총 15 개의 모형을 제작하였고 이를 인상 채득하여 작업 모형을 제작하였다. 각 모형에 측정용 지그를 연결하여 기준 모형 및 작업 모형 상의 거리를 비교하였다. 세 가지 실험군에서 각 임플란트 유사체(analog) 간 거리 차이는 통계적으로 유의하지 않았다(P>.05). 동일 수평면에 대한 수직적 거리를 비교한 결과 flag type의 인상용 코핑을 사용하여 제작한 작업 모형이 Point 2에서는 기준 모형에 가장 근접한 수치를 보였고 Point 3에서는 오차가 가장컸으며 이는 통계적으로 유의하였다(P<.05). 그 외 Point에서는 세 가지 방법간에 통계적으로 유의한 차이는 없었다(P>.05). 새로운 인상용 코핑을 사용한 인상법은 기존의 인상용 코핑을 사용한인상법과 비교적 유사한 정확성을 보여주었다.

Impression is one of the most important processes to reproduce patient's oral condition. Impression procedure is cannot emphasized in implant prostheses to much because they need more accurate process for success. It is known that pick-up impression technique is more accurate than transfer impression technique in multiple implants case. But it can be a causative of inadequate working cast because of self-curing acrylic resin for splinting impression copings is possible to occur linear shrinkage. The purpose of this study was to examine the availability of the new impression coping by comparing with conventional coping in implant pick-up impression technique. Five implant fixtures were installed on #14, 21, 23, 25, 27 area keeping parallelism between the fixtures using the parallelometer. That simulator with 5 fixtures was standard model, which was divided 3 groups; using new flag type impression coping, conventional impression coping splinted with self-curing resin, conventional impression coping without splinting. We made metal circular cones for calculation 3-dimensional coordinates by attaching to implant fixtures or analogs. Three-dimensional relationships of each model were calculated. Data was analyzed by multiple ANOVA and Bonferroni. The accuracy of impression between using new flag type impression coping and conventional impression coping did not show differences in 3 –dimensional analysis. Within limitations of this study, the new flag type impression coping is available in implant pick-up impression technique.
