양극성 장애 환자들은 정상 기분안정기에도 사회인지를 비롯한 다양한 인지기능 뿐만 아니라 정서 처리와 조절에도 장애가 있다고 알려져 있다. 그러나 양극성 장애 환자들이가상현실에서 상대방의 대화를 주의 깊게 듣고 추론하는 마음추론 과제 동안 관련되는 뇌 부위를 조사한 연구는 없었다. 본 연구는 기분안정기 양극성 장애 환자에서 정서부하에 따른 인지처리와 그 기저의 신경 기전을 조사하였다. 14명의 기분안정기 양극성 환자와 14명의 정상인들에게 기쁜, 화난, 중립정서 상태의 가상현실 아바타가 제시하는 대화로 구성된 모호한 과제(검사 과제) 혹은 명확한 과제(대조 과제)를 듣고 추론하는 동안 기능적 자기공명영상술을 시행하였다. 이에 각기 다른 과제의 보고-듣는 과정과 추론 과정의 신경 활성반응과 과제 추론시의 정서부하의 영향을 조사하였다. 그 결과 정상인에 비해 양극성 환자들은 정서 상태로 전달된 모호한 과제와 정서중립 상태로 전달된 대조 과제의 차이에서 보고-듣는 동안에는 우측 중전두회에서, 추론 과정에서는 우측 하/중전두회와 뇌섬엽에서 활성 저하가 관찰되었다. 그러나 중립정서 상태의 모호한 과제와 대조 과제 사이에는 두 군 사이에 유의한 활성도 차이는 없었다. 이러한 결과들은 기분안정기 양극성 장애 환자에서 정서 부하가 대화 과제에서 추론 과정의 인지정보 처리의 장애를 일으키며 양극성 장애의 우반구 기능 이상의 가능성을 시사한다.

Fourteen euthymic bipolar patients and 14 healthy comparison subjects underwent functionalmagnetic resonance imaging while listening to and inferringfrom theambiguous(test)orsimple(control)speechtaskconversedby virtualavatars with happy,neutralor angry emotion.We assessed brain responsesassociated with thelistening and inferenceofdifferenttasksand alsotheimpactofemotionalloading onthetaskinference.Euthymicbipolar patients showed decreased activity in the rightmiddle frontalgyrus(BA9) during the listening process,aswellasin therightinferior/middlefrontal gyri(BA45/6)and the rightinsularlobe during the inferring process when contrastingbetweentheemotionalambiguousandnonemotionalcontroltasks. Incontrast,bipolarpatientsexhibitednoactivitychangeduringthelistening and inferring processeswhen comparing nonemotionalambiguoustocontrol tasks.Reducedactivityofrightinferior/middlefrontalgyriandinsularlobein euthymicbipolarpatientsascompared to healthy subjectsissuggestiveof cognitiveprocessingdeficitsduringtheinferringprocessundertheinfluenceof emotionalloading and probable righthemisphere dysfunction in euthymic bipolardisorder.
