초록/요약
본 연구는 국내 가정전문간호사의 실무 현장 경험을 심층적으로 탐색하고, 향후 가정전문간호사의 실무, 교육, 정책적 지원을 위한 기초자료를 제공하기 위해 수행된 질적연구이다. 연구참여자는 국내 의료기관（상급종합병원, 종합병원, 의원） 가정간호사업소에 소속되어 가정간호 실무를 수행하고 있는 가정전문간호사 8명이었다. 연구자료는 2026년 3월 18일부터 4월 24일까지 일대일 심층면담을 통해 수집되었으며, 귀납적 내용분석 방법으로 분석하였다. 연구결과, 가정전문간호사의 실무 경험은 ‘병원에서 가정으로 확장된 실무’, ‘질병을 넘어 환자와 가족의 삶까지 체휼하는 돌봄’, ‘가정간호 영역을 지켜내려는 소명’의 3개 주제, 6개 범주, 17개 하위범주로 도출되었다. 첫 번째 주제, ‘병원에서 가정으로 확장된 실무’에서 참여자들은 병원 밖 단독 실무에서 환자의 상태와 가정환경을 종합하여 살피고, 의료진과 협력적으로 소통하며 현장 상황에 대응하면서도, 단독 방문과 처치에 따르는 책임과 소진을 함께 경험하였다. 두 번째 주제, ‘질병을 넘어 환자와 가족의 삶까지 체휼하는 돌봄’에서 참여자들은 환자의 질병과 처치뿐 아니라 가족과 삶의 맥락을 포괄하는 총체적 돌봄을 수행하였고, 환자의 임종과 가족의 슬픔까지 지지하였으며, 환자의 사적 공간에서 전문직 경계를 둘러싼 윤리적 문제를 경험하였다. 세 번째 주제, ‘가정간호 영역을 지켜내려는 소명’에서 참여자들은 현장의 어려움을 충분히 반영하지 못하는 제도적 한계를 경험하면서도, 연대와 사명감으로 가정간호를 지켜가고자 하였다. 본 연구결과는 가정전문간호사의 실무가 병원 밖 가정이라는 확장된 실무 현장에서 환자의 질병 상태를 넘어 가족의 삶을 아우르는 돌봄과 가정간호에 대한 사명감을 통해 수행되는 통합적 실무임을 보여주었다. 이러한 결과를 기반으로 향후 가정전문간호사의 실무 역할을 반영하여, 보다 실제적인 업무 기준, 보상체계 및 안전지원체계의 마련, 전문간호사 교육과정 및 자격 취득 이후의 현장 중심 지속 교육 체계 수립, 지역사회 기반 의료체계 내 전문적 역할 정립을 위한 정책적 지원 전략이 필요할 것으로 사료된다.

This qualitative study aimed to explore in depth the practice experiences of advanced practice nurses （APNs） in home health care in Korea and to provide foundational data for their practice, education, and policy support. The participants were eight APNs working in home health care units affiliated with medical institutions, including tertiary general hospitals, general hospitals, and clinics, in Korea. Data were collected through one-on-one in-depth interviews from March 18 to April 24, 2026, and analyzed using inductive qualitative content analysis. The findings of this study were organized into three themes, six categories, and seventeen subcategories. The three themes were “Practice expanded from hospital to home,” “Care that embraces the lives of patients and families beyond illness,” and “A calling to preserve the field of home health care.” First, in “Practice expanded from hospital to home,” the participants comprehensively assessed patients’ conditions and home environments while practicing alone outside the hospital and responded to field situations through collaborative communication with medical staff, while also experiencing the responsibility and burnout associated with solo home visits and procedures. Second, in “Care that embraces the lives of patients and families beyond illness,” the participants provided holistic care encompassing not only patients’ diseases and treatments but also their families and life contexts. They also supported patients at the end of life and families in their grief and experienced ethical issues concerning professional boundaries within patients’ private spaces. Third, in “A calling to preserve the field of home health care,” the participants experienced institutional limitations that did not fully reflect the realities of practice, yet sought to preserve home health care through solidarity and a sense of mission. The findings showed that the practice of APNs in home health care constitutes an integrated practice carried out in the expanded practice setting of patients’ homes, encompassing care for patients and families beyond illness and a sense of mission toward home health care. Based on these results, it is suggested that more practice-relevant standards, reimbursement systems, and safety support systems should be developed to reflect the practice roles of APNs in home health care. In addition, field-based continuing education systems after certification and policy support strategies for establishing professional roles within community-based healthcare systems are needed.
