- 국문요약 - 사례 기반 KTAS 교육의 중증도 분류 역량 향상 효과 -질병 계통별 정확도, 과소·과대 오류 방향성 및 인식 수준 변화를 중심으로- 연구 목적: 본 연구의 목적은 사례 기반 KTAS(Korean Triage and Acuity Scale) 교육이 응급구조학과 학생의 중증도 분류 역량에 미치는 효과를 정확도, 오류 방향성, 자기 인식 수준의 지표를 통해 분석하고, 질병 계통별 특성에 따른 교육 효과의 차이를 규명함으로써 향후 중증도 분류 교육의 교수 전략 수립에 기초 자료를 제공하는 데 있다. 연구 방법: 연구 대상은 A 시에 소재한 B 대학교 응급구조학과 3학년 재학생 28명이었으며, 사전 평가–사례 기반 KTAS 교육–사후 평가의 순으로 연구를 진행하였다. 중증도 분류 역량 평가는 10개 질병 계통별로 유사한 난이도의 두 가지(A/B) 사례를 활용한 총 20개의 문항을 구성하였으며, 이를 통해 정확도와 과소 및 과대오류 방향성을 측정하였다. 질병 계통 및 문항별 교육 전·후 차이는 McNemar test를 실시하였다. 자기 인식 수준은 중증도 분류 역량과 관련된 주관적 인식을 평가할 수 있도록 리커트(Likert) 5점 척도의 10문항을 사용하여 측정하고, Wilcoxon signed-rank test로 분석했다. 연구 결과: 사례 기반 KTAS 교육 전·후의 중증도 분류 정확도를 분석한 결과, 전체 20개 문항의 평균 점수는 교육 전 7.57점에서 교육 후 9.93점으로 유의하게 상승했다. 질병 계통 및 문항별 정확도는 신경계, 근골격계, 심혈관계 문항에서 유의하게 향상되었다. 과소분류 오류는 신경계, 근골격계, 심혈관계, 소화기계, 정신건강계, 정신건강계 사례에서 유의하게 감소했다. 반면, 과대분류 오류는 모든 문항에서 교육 전·후 유의한 차이가 나타나지 않았다. 자기 인식 수준은 교육 후 5개 영역 10개 전 문항 및 총합 점수(31.18점→ 34.25점)에서 유의하게 향상되었으며, 특히 임상적 판단 영역의 ‘증상 및 활력징후 기반 판단’ 문항에서 2.79점에서 3.71점으로 가장 큰 향상 폭을 보였다. 결론 및 의의: 본 연구를 통해 사례 기반 KTAS 교육이 응급구조학과 학생의 중증도 분류 정확도 및 자기 인식 수준에 긍정적인 영향을 미치는 것으로 나타났다. 과소분류 오류는 신경계, 근골격계, 심혈관계, 소화기계, 정신건강계의 총 5개 질병 계통 일부 문항에서 통계적으로 유의하게 감소했다. 반면, 과대분류 오류는 교육 전·후 유의한 차이를 보이지 않았다. 본 연구는 중증도 분류 교육의 효과를 정확도뿐만 아니라 과소 및 과대분류 오류 방향성과 자기 인식 수준까지 평가하였다는 점에서 의의가 있다. 또한 질병 계통별 교육 효과와 오류 변화 양상을 확인함으로써 향후 사례 기반 KTAS 교육의 내용 구성과 교수 전략 수립을 위한 기초 자료를 제시하였다. 다만, 일개 대학 응급구조학과 학생을 대상으로 수행된 단일군 사전·사후 연구라는 점에서 연구 결과의 일반화에는 제한이 있다. 후속 연구에서는 대조군을 포함한 장기 추적을 통해 사례 기반 KTAS 교육의 효과를 보다 체계적으로 검증할 필요가 있다. 핵심어: 중증도 분류, 한국형 응급환자 분류 도구(KTAS), 응급구조학과, 사례 기반 교육, 과소 분류, 과대 분류

- ABSTRACT - The Effect of Case-Based KTAS (Korean Triage and Acuity Scale) Training on Improvements in Triage Competence -Focusing on Accuracy by Disease System, Direction of Under- and Over-Triage Errors, and Changes in Level of Awareness- Purpose: This study aims to evaluate the effects of case-based KTAS (Korean Triage and Acuity Scale) training on emergency medical services students' triage competencies, with indicators of accuracy, error directionality, and self-awareness. By identifying differences in training effectiveness in specific disease systems, the current study aims to lay the groundwork for future teaching strategies in triage education. Methods: The study participants consisted of 28 junior students in the Department of Emergency Medical Services at B University, located in City A. The study was conducted in the following sequence: pre-test, case-based KTAS training, and post-test. The assessment of triage competence consisted of a total of 20 items across two cases (A/B) of similar difficulty for each of the 10 disease systems, used to measure accuracy and the direction of under- and over-triage errors. Differences before and after training, by disease category and item, were analyzed using the McNemar test. Self-perception levels were measured using 10 items on a 5-point Likert scale to assess subjective perceptions of triage competence, and were analyzed using the Wilcoxon signed-rank test. Results: An analysis of triage accuracy before and after case-based KTAS training revealed that the average score across all 20 items increased significantly from 7.57 before training to 9.93 afterward. Accuracy by disease system and item showed significant improvement in items related to the nervous, musculoskeletal, and cardiovascular systems. Under-triage errors decreased significantly across the nervous, musculoskeletal, cardiovascular, digestive, and mental health systems. In contrast, no significant difference in over-triage errors was observed between pre- and post-training scores across all items. The level of self-awareness improved significantly after the training across all 10 items in the five domains and in the total score (31.18 points → 34.25 points); in particular, the item “Judgment Based on Symptoms and Vital Signs” in the clinical judgment domain showed the greatest improvement, rising from 2.79 points to 3.71 points. Conclusion: This study demonstrated that case-based KTAS training has positive effects on triage accuracy and self-awareness among emergency medical services students.Under-triage errors decreased statistically significantly in some items across five disease systems: neurological, musculoskeletal, cardiovascular, gastrointestinal, and mental health. However, no significant difference in over-triage errors was observed before and after the training. This study is significant because it evaluated the effectiveness of triage training not only in terms of accuracy but also in terms of the directionality of under- and over-triage errors and the level of self-awareness. Furthermore, by identifying the effects of the training and the patterns of error changes by disease system, this study provided foundational data for structuring the content and developing teaching strategies for future case-based KTAS training. Nevertheless, as this was a single-group pre-post study conducted with students in a single university’s emergency medical services program, the results are limited in generalizability. Future studies should use long-term follow-up, including a control group, to more systematically evaluate the effectiveness of case-based KTAS training.
