- 국문 요약 - 응급실 간호사의 환자안전 침묵행동에 미치는 영향요인 본 연구는 응급실 간호사의 환자안전 발언분위기, 전문직 자율성이 환자안전 침묵행동에 미치는 영향요인을 확인하기 위해 시행된 서술적 조사연구이다. 본 연구는 경기도 소재 상급종합병원 1개소, 종합병원 1개소 각각의 응급의료센터에서 근무하는 응급실 간호사 중 응급실 근무 경력 6개월 이상이며 직접 간호를 제공하는 간호사 125명을 대상으로 하였다. 자료수집은 2025년 07월 12일부터 2025년 9월 30일까지 구조화된 설문지를 이용하여 수행하였으며, 수집된 자료는 IBM SPSS Statistics 21.0 통계 프로그램을 이용하여 자료는 빈도와 백분율, 평균과 표준편차, Independent t-test, one-way ANOVA, Pearson’s correlation coefficients 및 multiple regression을 이용하여 분석하였다. 본 연구의 결과는 다음과 같다. 대상자의 환자안전 발언분위기는 평점 평균 3.82±0.63점(5점 만점), 전문직 자율성은 평점 평균 3.25±0.54점(5점 만점), 환자안전 침묵행동은 평점 평균 3.88±0.65점(5점 만점)이었다. 침묵행동은 최종학력(F=3.20, p=.044), 현재 직위(F=3.58, p=.031), 현 부서경력(F=3.41, p=.020)에 따라 통계적으로 유의한 차이를 보였으며, 침묵행동은 환자안 전 발언분위기(r=.38, p〈.001), 전문직 자율성(r=.35, p〈.001)과 유의한 양의 상관관계를 보였다. 회귀분석 결과, 환자안전 발언분위기(β=.27, p=.003)와 전문직 자율성(β=.20, p=.028)가 침묵행동에 영향을 미치는 주요 요인으로 확인되었으며, 이들 변수의 전 체 설명력은 25.7%였다(F=4.42, p〈.001) 본 연구 결과, 환자안전 발언분위기와 전문직 자율성은 응급실 간호사의 환자안전 침묵행동에 영향을 미치는 주요 요인으로 확인되었다. 이를 토대로 응급실 간호사의 환자안전 침묵행동을 감소시키기 위해서는 단순히 발언을 장려하는 수준을 넘어, 위계적 의사소통 구조와 조직문화를 개선하고 안전한 의사표현 환경을 조성하기 위한 조직 차원의 중재 전략이 필요하다. 또한 응급실의 특수한 업무 환경을 고려하여 환자안전과 관련된 의견 제시가 업무 흐름의 방해가 아닌 안전한 의사소통 과정으로 인식될 수 있도록 조직 차원의 지원과 체계적인 의사소통 교육이 필요하다.

-ABSTRACT- Factors Influencing Patient Safety Silence Among Emergency Nurses This study is a descriptive correlational study conducted to identify the levels of speaking-up climate for patient safety, professional autonomy, and patinet silence behavior in emergency department nurses and to examine factors influencing silence behavior. Data were collected using a structured questionnaire from nurses who had worked for at least six months and provided direct patient care at two emergency medical centers, including one tertiary general hospital and one general hospital located in Gyeonggi Province, in total 125 nurses. The participants included nurses capable of effective communication and performing clinical duties. Patient safety speaking-up climate was measured using the Speaking Up Climate for Patient Safety Scale developed by Martinez et al. (2015) and translated into Korean by Ji and Lee (2022). Professional autonomy was assessed using the Professional Autonomy Scale developed by Schutzenhofer (1983), translated by Han et al. (1994), and subsequently modified into an 11-item version for nurses by Kim (2005). Silence behavior was evaluated using the instrument developed by Tangirala and Ramanujam (2008) and modified by Han (2017). The collected data were analyzed using IBM SPSS Statistics 21.0, with descriptive statistics, Independent t-tests, one-way ANOVA, correlation analysis, and multiple regression analysis performed. The major findings of this study were as follows: 1. The mean score for speaking-up climate for patient safety among the participants was 3.82±0.63 out of 5 points, the mean score for professional autonomy was 3.25±0.54 out of 5 points, and the mean score for patient safety silence behavior was 3.88±0.65 out of 5 points. 2. Significant differences in patinet silence behavior were found according to educational level (F=3.20, p=.044), current position (F=3.58, p=.031), and length of work experience in the current department (F=3.41, p=.020). 3. Silence behavior was positively associated with patient safety speaking-up climate (r=.38, p〈.001) and professional autonomy (r=.35, p〈.001). In addition, patient safety speaking-up climate was positively associated with professional autonomy (r=.38, p〈.001). 4. Factors influencing silence behavior were patient safety speaking-up climate (β=.27, p=.003) and professional autonomy (β=.20, p=.028). The findings of this study identified speaking-up climate for patient safety and professional autonomy as major factors influencing patient safety silence behavior among emergency department nurses. In other words, emergency department nurses who perceived a more positive speaking-up climate for patient safety and reported higher professional autonomy paradoxically demonstrated higher levels of patient safety silence behavior. Based on these findings, reducing patient safety silence behavior among emergency department nurses requires organizational-level interventions beyond simply encouraging staff to speak up. Efforts are needed to improve hierarchical communication structures and organizational culture and to create a safe environment for expressing patient safety concerns. In addition, considering the unique characteristics of emergency department settings, organizational support and systematic communication training are necessary so that expressing patient safety concerns is recognized as part of safe clinical communication rather than as an interruption to workflow.
