서론: 본 연구의 목적은 과개교합 환자의 교정 치료 시 기존의 고정식 교정장치와 Invisalign을 사용할 때의 하악 전치부 이동 양상을 비교하는 것이다. 재료 및 방법: 과개교합 환자의 교정 치료에서 고정식 교정장치와 인비절라인을 통한 치료 시 하악 전치부 이동 양상에 관한 연구 서 론: 본 연구의 목적은 과개교합 환자의 교정 치료 시 기존의 고정식 교정장치 와 Invisalign을 사용할 때의 하악 전치부 이동 양상을 비교하는 것이다. 재료 및 방법: 과개교합을 가지고 있는 47명의 부정 교합 환자(남: 14명, 여: 33명, 평균 연령: 29.8 세)를 대상으로 하였다. Invisalign 또는 고정식 교정장치를 통한 교 정 치료(Invisalign: 21명, 고정식 교정장치: 26명)를 진행했으며, 치료 전 후 측모두 부방사선 사진을 촬영하여 측모두부방사선 계측치 분석을 통해 Invisalign과 고정식 교정장치 치료의 결과를 비교하였다. 하악 전치부의 이동 양상을 세분화하여 분석 하기 위해 하악 평면과 이에 수직인 선을 기준으로 하여, Crown tip, Centroid, Root tip 에서의 이동을 2차원적으로 분석하였다. 결과: 치료 전후 결과를 비교한 결과, Invisalign 환자군과 고정식 교정장치 환자군 모두 수직 고경의 변화 없이 하악 전치의 수직적인 함입을 통한 과개교합의 개선 이 일어났다. Invisalign 환자군에서는 하악 전치부의 약간의 순측 경사와 함께 교합 평면의 시계 방향 회전이 일어났다. 고정식 교정장치 환자군은 하악 전치의 전후 방적 이동 양상이 다양하게 나타나, 발치 여부로 세분화하여 비교를 진행하였다. 그 결과 고정식 교정장치에서 발치를 진행하지 않은 Fixed Group-Non Ext (FG- NonExt) 군과 Invisalign 환자군은 하악 전치의 이동 양상에서 통계적으로 유의미한 차이를 보이지 않았다. 결 론: 성인 과개교합 환자에서 수직 고경을 유지하고 싶을 경우, Invisalign은 기존 의 고정식 교정 장치의 충분한 대안이 될 수 있으며, 더 나아가 미니스크류 등의 부가적인 도움 없이도 원하는 결과를 얻을 수 있다.

Objectives: The study aims to define the lower anterior teeth movement after deep overbite correction by Invisalign and conventional fixed orthodontic treatment. Materials and Methods: The 47 patients (male: 14, female: 33, mean age: 29.8 years) diagnosed with deepbite were included. All of the patients went through orthodontic treatment by either Invisalign or conventional fixed appliance (Invisalign: 21, fixed appliance: 26). Lateral cephalograms were taken at the time of initial and final stage. Cephalometric measurements were analyzed and compared. To specify lower anterior teeth movement, we used mandibular plane and the line perpendicular to it as a reference line and analyzed lower anterior teeth movement. Results: All of the patients’ deepbite was well treated and improved by the pure intrusion of lower incisors as well as maintaining the vertical dimension. Labioversion of lower incisors and clockwise rotation of occlusal plane were noted in Invisalign Group. For the Fixed group, lower incisor movement showed different patterns, so we divided the patients by whether extraction was accompanied or not. For the lower incisor movement, Fixed Group-NonExt (FG-NonExt) and Invisalign Group did not show statistical difference between two groups. Conclusions: To remain vertical dimension in adult deepbite patients, Invsialign could be a great alternative for conventional fixed appliance. Moreover, we can even get desirable outcomes without using auxiliaries such as TADs while using Invisalign.
