cAMP 신호 활성에 따른 이독성 난청 예방의 증강약물 발굴

초록/요약

배경 및 목적: 이독성 난청은 특정 약물의 부작용으로 내이의 달팽이관 세포에 기능적 손상이 발생하는 증상이다. 본 연구에서는 항암제 시스플라틴 (cisplatin, CDDP)에 의한 이독성 난청의 예방을 목적으로 하였으며, 인도 식물의 뿌리에서 추출한 천연물 포스콜린 (forskolin, FSK)과 비타민 A의 최종 산물인 레티노산 (retinoic acid, RA)을 후보 약물로 선정하였다. CDDP로 인한 이독성이 유발되었을 때, FSK 및 RA 약물의 1) 청력손실 보호, 2) 내이의 달팽이관 세포 보호, 3) 관련 기전을 평가하는 것이 본 연구의 목적이다. 또한 두 약물의 단독 투여보다 복합하여 투여했을 때 증강된 효과를 보일지를 평가하였다. 대상 및 방법: 동물은 수컷 7~8주령 Sprague-Dawley (SD) rat을 사용하였다. 실험동물은 대조군 (Control), CDDP+Normal Saline 투여군 (CDDP+N.S), CDDP+Forskolin 투여군 (CDDP+FSK), CDDP+Retinoic acid 투여군 (CDDP+RA), CDDP+FSK+RA 투여군 (CDDP+FSK+RA)의 다섯 군으로 나누어 진행하였다. CDDP는 복강내 투여를 하였고, N.S, FSK, RA는 고실내 주입으로 CDDP전후로 3일간 투여하였다. CDDP투여 후 5일 뒤에 청성뇌간반응 검사 (auditory brainstem response test, ABR)를 통해 청력을 측정하여 그룹에 따른 변화를 비교하고, 달팽이관을 채취하여 Hematoxylin & Eosin 및 면역 염색, cochlear whole mount 등을 실시하였다. 결과: 실험은 이독성 난청 동물모델을 확립하는 것부터 시작하였다. CDDP로 인한 청력감소 및 면역조직검사에서 산화 스트레스와 활성산소에 의해 유도되는 AMPK의 인산화 증가와 와우 외측벽에서 Na+, K+ 이온 조절에 관여하는 효소인 Na+ / K+ ATPase α1의 발현 감소를 확인하였다. FSK 및 RA의 단독 및 복합 투여는 CDDP로 인한 청력소실을 예방하였고, 달팽이관의 유모세포 (hair cell, HC), 나선신경절 세포 (spiral ganglion cell, SGC), 와우 외측벽 (lateral wall, LW) 세포의 소실 예방효과를 보였다. Cochlear whole mount 염색을 통해 CDDP+N.S과 CDDP+FSK+RA 군간의 유모세포 핵의 수를 측정하였을 때, CDDP+N.S 군에 비해 CDDP+FSK+RA 군의 중간부 (middle turn)에서 남아있는 핵의 수가 유의하게 많았다. 또한 CDDP 투여에 의해 감소된 p-PKA와 p-CREB 발현이 약물 투여에 의해 증가함을 통해 FSK과 RA의 세포 보호 효과에 cAMP/PKA/CREB 신호가 관여함을 알 수 있었고, 간극결합 활성효과의 RA와 FSK의 복합투여에 의해 관련 신호 증강 효과를 나타냈다. 결론: 본 연구를 통하여cAMP 및 간극결합에 의한 세포간 신호전달 메커니즘을 타깃으로 하는 두 약물 FSK과 RA는 CDDP유도 이독성 난청 예방의 후보 약물로서 가능성이 있을 것으로 사료된다.

Purpose : Ototoxicity causes functional damage to the cochlear cells of the inner ear due to the side effects of certain drugs. The purpose of this study was to prevent ototoxicity caused by cisplatin (CDDP), one of the anti-cancer drugs. A cyclic adenosine monophosphate (cAMP) stimulator, forskolin (FSK) and a gap junction enhancer, retinoic acid (RA) were selected as candidate drugs. To confirm the preventive effect of FSK, RA and combination on CDDP-induced hearing loss in animal models, we evaluated the 1) hearing function, 2) cochlear cell viability and 3) mechanisms involved in cAMP/PKA/CREB and gap junctions the combined use of FSK and RA would produce a enhanced protective effect. Materials & methods : 7-8 week old male Sprague-Dawley (SD) rats were used. Rats was divided into 5 groups: Control group (Control), CDDP+Normal Saline group (CDDP+N.S), CDDP+FSK group (CDDP+FSK), CDDP+RA group (CDDP+RA), and CDDP+FSK+RA group (CDDP+FSK+RA). N.S, FSK and RA were administered through tympanic membranes for 3 days before and after CDDP and CDDP was administered intraperitoneally once. After 5 days, the hearing was measured by auditory brainstem response test (ABR). The cochlea was collected to evaluate hematoxylin & eosin staining and immunohistochemistry. Result : Rats were intraperitoneally injected with CDDP and showed the increase in hearing thresholds with cochlear cell damage. FSK and RA treatment significantly reduced hearing thresholds, and the protective effect was significantly higher in the combination than single use. CDDP-induced loss of cell and shrinkage of nuclei were observed in hair cells (HC), spiral ganglion (SG), and lateral wall (LW) cells. On the other hand, FSK + RA group showed reduced cell loss compared to CDDP+N.S group as well as the FSK and RA alone groups. When comparing the number of nuclei of HCs in the CDDP+N.S and CDDP+FSK+RA groups, a large number of cells were observed in the CDDP+FSK+RA group, especially in the middle turn. Immunostaining revealed an increase of p-PKA, p-CREB, Na+ / K+ ATPase α1, with decreased p-AMPKα levels in FSK and RA-treated rats. Furthermore, the combined administration of these two drugs showed a more enhanced effect on the levels of proteins when compared to the single administration. Conclusion : These results suggest that cAMP signaling and gap junctional intercellular communcation could be a promising strategy for preventing CDDP-induced ototoxicity.
