연명의료중단 노인돌봄이 요양보호사의 윤리적딜레마와 직무스트레스에 미치는 영향

본 연구는 노인요양시설에서 연명의료중단 노인의 임종 돌봄에 대한 요양보호 사의 경험과 인식이 윤리적 딜레마와 직무스트레스에 미치는 영향을 검증하기 위함이다. 향후 연명의료중단에 따른 존엄사에 대한 이해와 노인요양시설에서 의 호스피스 임종교육, 표준화된 임종 돌봄 지침서 및 프로그램개발의 방향을 제시하는데 목적이 있다. 이를 위한 본 연구문제는 다음과 같다. 첫째, 요양보호사의 연명의료중단 노인 돌봄의 경험과 인식은 어떠한가? 둘째, 연명의료 중단노인 돌봄 경험이 요양보 호사의 윤리적 딜레마에 미치는 영향은 어떠한가? 셋째, 연명의료 중단노인 돌 봄 경험이 요양보호사의 직무스트레스에 미치는 영향은 어떠한가? 본 연구를 위해 경기도 수원시와 화성시에 소재한 노인요양시설의 20개 기관 에 근무하고 있는 요양보호사 325명 대상으로 조사연구를 실시하였다. 조사대 상자 및 자료의 특성을 파악하기 위해 빈도분석 및 기술통계 분석과 연명 의료중단결정 노인 돌봄이 윤리적 딜레마와 직무스트레스에 미치는 영향을 검증하기 전해 각 변수들 간의 상관관계를 분석 한 후 다중회귀분석을 실시하였 다. - ii 본 연구의 결과를 요약하면 다음과 같다. 첫째, 연명의료중단 노인돌봄경험과 시행인식의 필요성은 79.1%가‘그렇다’ 로 대부분의 요양보호사들은 연명의료 중단이 필요하다고 생각했다 둘째, 연명의료중단 노인 돌봄이 윤리적 딜레마에 미치는 영향을 살펴보면 성 별이 여자이고, 학력이 높고, 근무기간이 길수록, 연명의료중단 돌봄 시기가 최 근일수록, 돌봄 인원이 적을수록, 돌봄 어려움정도가 클수록, 교육경험이 없을 수록 윤리적 딜레마는 높은 것으로 나타났다. 셋째, 연명의료 중단노인 돌봄이 직무스트레스에 미치는 영향을 살펴보면 연령 이 많을수록, 근무기간이 길수록, 연명의료중단 돌봄 시기가 최근일수록 돌봄 인원이 적을수록, 돌봄 어려움이 클수록, 교육경험이 없을수록 직무스트레스가 높은 것으로 나타났다. 이러한 연구 결과의 내용을 바탕으로 향후 연명의료중단 노인 돌봄 시 윤리적 딜레마와 직무스트레스를 줄이기 위해 다음과 같이 제언을 하고자 한다. 첫째, 연명의료중단 노인 돌봄이 윤리적 딜레마에 미치는 영향에서 교육을 받 은 사람일수록 윤리적 딜레마와 직무스트레스가 낮았으므로 요양원의 필수교육 인 급여제공지침교육에 호스피스 임종과정에 대한 교육을 포함하여야 한다. 정 기적으로 보수교육을 받을 수 있도록 법적으로 제도화하여야 한다. 둘째, 요양보호사들이 임종 돌봄의 어려움 정도는 윤리적 딜레마와 직무스트레 스에 유의미 하였으므로 임종노인 돌봄에 심리적인 두려움을 느끼는 요양보호 사를 위한 체계적이고 표준화된 임종 돌봄 지침서 및 프로그램을 개발하여야 할 것이다. 셋째, 연명의료중단 노인돌봄경험시기가 최근일수록 요양보호사의 윤리적 딜레 마와 직무스트레스에 유의미하였으므로 임종 돌봄을 제공한 요양보호사들에게 는 전문적인 심리 상담사에게 심리 상담을 제공해야 한다.

The purpose of this study is to verify the effect of the caregiver's experience and perception on the dying care of the elderly at the elderly care facility on ethical dilemmas and task stress. The aim is to present an understanding of future death with dignity due to the suspension of life care and direction of development of hospice death education, standardized death care guidelines and program development in elderly care facilities. To this end, this research issue is as follows. First, what about the experience and awareness of the nursing care worker's care for the elderly? Second, what is the impact of the senior care experience on the ethical dilemma of nursing care providers? Third, what is the effect of the care experience on the work stress of the caregiver on the caregiver? For the purpose of this study, 20 institutions of elderly care facilities located in Suwon and Hwaseong, Gyeonggi Province A survey was conducted on 325 medical care providers who were not allowed to do so. To identify the characteristics of the subjects and data, multiple regression analysis was performed after analyzing the correlation between the variables before frequency analysis and analysis of descriptive statistics and the effect of care for the elderly who decide to discontinue life on ethical dilemmas and task stress was verified. In summary, the results of this study are as follows. - 60 First, 79.1% of senior care experience and the need for awareness of practice and care for senior citizens were 'yes' and most caregivers thought it was necessary to discontinue life care. Second, looking at the impact of care on ethical dilemma of senior citizens who are suspended from life care, the higher their gender, the higher their educational background, the longer their working days are, the fewer the number of caregivers, the greater the educational experience, the higher the ethical dilemma. Third, if you look at the impact of care on the job stress of senior citizens who are suspended from life care, it turns out that the older the age, the longer the working period, the fewer the number of caregivers in recent years, the greater the difficulty of care and the less education experience, the higher the task stress. Based on the results of these studies, the following suggestions are made to reduce ethical dilemmas and task stress in the future care of senior citizens who are out of life care. First, the education on the course of death in hospice should be included in the education of wage guidelines, an essential education in nursing homes, as the person who was educated in the impact of the caregiver's care on the ethical dilemma was less ethical dilemma and task stress. It should be legally institutionalized so that it can receive regular maintenance education. Second, the degree of difficulty of death care. care by nursing care providers is significant to the ethical dilemma and task stress, so they should develop a systematic and standardized death care guideline and program for nursing care workers who feel psychological fear during the care of the elderly. Third, as the experience of care for senior citizens who have stopped living medical care has become more recent, the nursing care providers' ethical dilemma and task stress are more important to them, so they should provide counseling to professional psychologists.
